The goal of a gameplay session in Ai-SICS is to successfully perform small incision cataract surgery on a patient. This is a simulator grade application, so every aspect of the game aims to be as realistic as possible.

A patient can be randomly generated, or customised by the user. Every detail is customisable, from eye colour and skin colour, to severity of cataract and eye moisture.

The user has a standard set of surgical tools for this type of cataract surgery at their disposal, and must use them as they see appropriate to perform the operation. The steps they take, and the precision in which they use them will affect the outcome of the operation, with all consequences (good or bad) replicated as they would happen in the real world.

Upon completing the surgery, the user and tutors will be able to evaluate every step that was taken.
